I am encountering errors when attaching files o documents.  It is random and does not appear to be based on the size of the attachment.  Myself and a colleague have experienced this.  

All of the documents that have done this have allowed us to attach the file if we go back to the item some time later/the next day.  However this is adding time to an already manual and time consuming process that we are plodding through as we set up our QMS for the department.  Ideally we need to be able to create all the documents and attach the files at the same time as we create them instead of having to go back to some to attach.
