Hi ​@Amanda Lepley, the reports in the Occurrence module are completely unique to your environment.

As you build an occurrence form you’ll have unique fields.  These fields need to be created on the report.

The easiest way to do this is to edit the blank report and then add a table.

You can then add the number of columns you need and then drag and drop the fields from the field list into the table cells. You can find some details on this at Adding tables to reports – Quality Management (Professional)

After posting this, I then sat and worked on it and managed to build the three I wanted initially.  Thank you for responding.
