Hi ​@kamila.lyczek.  That’s an interesting idea.  At the moment you can’t add a risk matrix to the form itself as this is something that’s available when you perform the review as part of the incident investigation.

However, you could create a few fields on the form for the impact and severity to be filled in on the form as this would encourage the raiser to think about the risk impact. The user who investigates the incident can then take these values and update the incident risk assessment.

Thank you for you answer Alan , another feature would be nice to have within the forms is to be able to change the background colour when you split the sections of the form . 

Hi Kamila, it’s not something that’s possible just now and I’m not sure if this would be possible in the technologies used to build the forms but as the product evolves this may be something that could be introduced.  I’ll pass your feedback on to our product team.
