When a document was issued out for approval one of the approvers highlighted the incorrect name and was able to reject the document with comment and save

Is there a way to lock it so that they are only able to complete the approve/reject against their own name

I don’t want to end up in a scenarios where by one person can go in and approve/reject for everyone

Any advise would be appreciated

I don’t know if it is possible to limit the response to their own name, however, there is a way to record who performed the action. 

Go to Administration module → Security → Signature Management; In the list, find “Document Respond” (might be named differently by your system admin) and tick the box next to it. 

This means that when Approving or Rejecting a document, a user will be prompted to enter their login details as a way of “signing it”. 

Where this is done on behalf of another user, a note will come up in the date section, like in the screenshot below. 

It enables you to see who’s response is recorded.

What you’re describing sounds like a permissions-related issue. It’s likely that the "Document Respond" permission has been granted via a static security group, which would allow users to approve or reject documents on behalf of others.

To prevent this, I’d recommend switching to the dynamic security group named "Document Approver". Assign the necessary permissions to that group instead. This setup ensures that users can only respond to documents where they are explicitly named as an approver, avoiding the scenario where one person can action approvals or rejections on behalf of others.
